How should we interpret the word “access” in relation to the billing of item 14221?

Context

Item 14221 - question regarding the definition of “access”
Is access defined as inserting the Huber needle, or does access refer to each instance of the administration of different therapeutic agents (not associated with item 13950) at different times over the course of a multiday admission) e.g. over the course of the admission IV fluid, blood products, antibiotics etc are administered at different times by connection to the Huber needle?
How should the doctor bill if a patient receives more than 1 therapeutic agent at different times on the same day via a portacath?
