Can an ophthalmologist claim item 111 in circumstances where the planned operation may need to change on the day of surgery?

Context

As an ophthalmologist, if I have planned a surgical procedure for my patient, but on the day of the procedure, while speaking with the patient pre-operatively, it become apparent that they will also need something additional done to that eye, does this qualify to bill item 111 because it was an unexpected procedure?
